(Summary of the daily activities)
Individual is diagnosed with cerebral palsy and quadriplegia. His home has been outfitted with a Hoyer lift and a sidelyer (Caregiver will be trained). Individual uses a wheelchair (manual and power) to get around his home and in the community. He relies on maximum and total assistance for every aspect of his life, including all ADLs, (dressing, bathing, toileting, oral hygiene, etc.) However, he is encouraged to participate as much as possible in order to foster his independence. He needs complete and total assistance in transferring to the toilet and with cleaning. Caregivers prepare, cook and feed him his meals; however, he is encouraged to feed himself approximately 50% of the time, several times per day. Some meals may already be prepared by his mom. He wants to be "on the go" and out in his community as much as possible. His primary focus remains as getting out and accessing his community. Caregiver will use Access Ride to transport him to and from the community; The Caregiver will call and arrange for pick-up and assist with getting him on and off the van/bus. Most of the time he is out in the community during the day hours and home by 3pm to meet the next Caregiver. Individual can make minor day to day decisions in his life; what to wear, what eat, where to go, and what he wants to watch on television. He makes these decisions known by verbalizing or making gestures.
